Giant cell tumor of the upper dorsal spine extending as a huge mediastinal mass, - a challenging case for management

Abstract


Abstract : AbstractIntroduction Gaint cell tumour usually arises from long bones. Only 2-4 GCT occurs in thoracic spine , but GCT of dorsal spine extending into mediastinum as huge mass is extremely rare.Case report Here we present a case of GCT 3rd dorsal vertebrae in a 25 year old female presented with history fall with paraplegia after one month . X ray showed reduction in height of D3 and opacity of left upper thorax. CT and MRI revealed destruction of posterior part of 3rd rib and presence of huge mass in left thorax extending upto the level of D 7,and cord compression at level of D3.MethodsA diagnosis of gaint cell tumour of dorsal spine with paraplegia was made. Through left posterolateral             thoracotomy, chest cavity opened between third and fourth rib. Hard encapsulated mass exposed. D3 spondylectomy with resection of 3rd rib and mediastinal mass and anterior stabilisation was done . Results patient showed recovery of pain and deep touch after 3 month follow up.Conclusion GCT of upper dorsal spine extending into upper medistinum as huge mass is extremely rare. Early diagnosis and intervention should be done to prevent neurological damge .Hence this case need to be reported.
