Intraperitoneal Haemorrhage from a vessel on the surface of a Uterine Fibroid

Abstract


Background - Intraperitoneal Haemorrhage
from uterine fibroid is rarely encountered.
This article describes spontaneous
intraperitoneal bleeding from a
ruptured vessel on the surface of uterine
fibroid. Case - We encountered a 35-
year old multiparous woman who presented
with acute abdominal pain. She
had an acute abdomen, increasing pallor
and a pelvic mass with free fluid in the
abdomen and pelvis. Laparotomy was
performed and revealed 15 x 18 cm pedunculated
sub serous fibroid with ruptured
surface vein which was actively
bleeding with 2000ml of hemoperitoneum.
Hysterectomy was performed. Patient
required blood transfusion, but otherwise
did well. Conclusion -This case
suggest that, vessel rupture on the surface
of Uterine Fibroid, though a rare
complication should be considered in patients
presenting with Intraperitoneal
Bleeding and Pelvic Mass.
