Anaesthetic Management Of A Patient With Pheochromocytoma Posted For Left Adrenelectomy

Abstract


Pheochromocytoma is  a catecholamine secreting tumor of primitive neural crest occurs in adrenal medulla and paraganglia of sympathetic nervous system. It can pose wide hemodynamic instability during perioperative period. This is a case report of a 46 year old lady diagnosed to have pheochromocytoma posted for left adrenelectomy. After having adequate preoperative preparation with prazosine ,propranalol,IV fluids open surgical removal of tumor was done. Intraoperative hemodynamic fluctuations are managed with IV nitroglycerine,IV labetalol ,noradrenaline infusion. Postop recovery was uneventful. Blood pressure becomes normal on 1st postoperative day. catecholamine secreting tumors have wide fluctuations in BP intraoperatively. Careful preoperative optimization ,intraoperative planning and hemodynamic management is necessary to prevent complications.
