Rare case report of primary pulmonary hodgkins lymphoma

Abstract


Primary pulmonary Hodgkin Lymphoma is an extremely rare disease and only around 100
cases has been reported in literature worldwide so far. Here we present a case of PPHL seen in our
hospital. A 32 year old female, non smoker, presented with a complaint of dry cough of 2 months
duration associated with chest pain which did not resolve with routine treatment. She had occasional
low grade fever but no significant loss of weight. Her routine Blood counts were normal and her renal
and hepatic parameters were also normal. Her CT chest revealed a large ill defined heterogeneously
enhancing soft tissue density lesion in the apicoposterior and anterior segments abutting the
mediastinal great vessels. There was no evidence of enlarge nodes. A thoracotomy and biopsy was
done and it revealed lymphoid aggregates with occasional Reed-Sternberg cells. IHC was positive for
CD15, CD30, EMA and PAX5 which was consistent with classical Hodgkins Lymphoma nodular
sclerosing type. Bone marrow examination was done which did not show and abnormality and further
imaging failed to show any other disease in the body. The final diagnosis was Primary Pulmonary
Hodgkin Lymphoma. The patient is presently undergoing chemotherapy. This case is being presented
for its rarity.
