Rare presentation of a solitary mid brain Neurocysticercosis causing recurrent third nerve palsy

Abstract


Neurocysticercosis, a common tropical parasitic disease uncommonly causes third nerve palsy and                recurrence of the same is very rare. We present a case of mid brain neurocysticercosis causing recurrent third nerve palsy of 6 episodes. Each episode on treatment with antihelminthics and steroids resulted in complete recovery only to recur after a period of stopping the treatment. The recurrences occurred even after complete calcification on MRI. Our experience with this patient suggests that recurrences can occur even beyond the nodular calcified stage and requires prompt treatment with steroids with gradual tapering to avoid any residual effects.
