A Case Report of Papillary Carcinoma Thyroid with Hematogenous Spread and Rare Soft Tissue Metastases.

Abstract


Follicular thyroid carcinoma is known to have  hematogenous spread and give rise to bone and lung  metastases.Such a pattern of spread is rare in papillary thyroid carcinoma. The follicular variant of papillary thyroid carcinoma is believed to behave similar to classical      papillary cancer. There are few reports of aggressive         follicular variant of papillary thyroid cancer wherein           follicular patterned tumors with nuclear features of papillary carcinoma have metastasized hematogenously. Here I am reporting a case of follicular variant of papillary carcinoma thyroid with hematogenous metastases. A 65 year old male admitted with complaints of thyroid swelling and left sided cheek swelling was evaluated and diagnosed to have thyroid malignancy with soft tissue metastases. On imaging the patient was found to have multiple bone, lung and brain metastases. Total thyroidectomy followed by radioactive iodine ablation was done. Histopathological examination of specimen revealed follicular variant of  papillary carcinoma thyroid. Patient is in follow up with radiotherapy for brain and bone metastases.
