Free functional Gracilis muscle transfer for elbow flexion animation in adult brachial plexus palsy

Abstract

Introduction Restoration of elbow flexion is an  important step in sequential management of brachial plexus Injuries to turn limb into a useful one. When the denervation time is more than one year or other procedures like Pectoralis major muscle and Latismus Dorsi muscle transfers were not possible because they were either paralyzed or ineffective, then gracilis muscle can be used as a free functioning muscle to bring about elbow flexion.

Aim The aim of this study was to evaluate the outcome of free gracilis muscle transfer for restoring elbow flexion.

Material and methods This is a retrospective study of free functioning gracilis muscle transfers 22 patients  (21 men and 1 woman) 21 patients had global brachial plexus palsy and    1 had upper trunk palsy.Their average age is 28.1 years. The surgical technique and rehabilitation protocol were the same for all the patients.

ResultsAverage time for obtaining M3 muscle power was 9 months.And for M4+ was 12.5 months.Two cases muscle necrosed due to venous thrombosis. Despite salvage attempt they could not be saved.
