Bipolar vs monopolar resection of prostate: a clinical and Biochemical comparison

Abstract


Background

Aim of this prospective clinical study was to compare clinical and biochemical outcome of Bipolar resection of prostate in normal saline [TURIS] and Monopolar resection of prostate in sterile water [CONVENTIONALTURP] under standardized     anaesthesia care.

 Methods

            A total of 60 patients were enrolled in this study.  Patients are divided into two groups by computer generated randomnization. All patients suffered from Benign Prostatic Hyperplasia and were scheduled for elective Transurethral Resection of Prostate under Spinal Anaesthesia. Group A patients underwent Bipolar resection in normal saline [TURIS].Group B patients underwent Monopolar resection in sterile water [CONVENTIONAL TURP]. Primary parameters observed were preoperative and postoperative Haemoglobin, serum sodium, IPSS score.Other parameters analyzed were time taken for resection and clot retention.

Results

            Systemic absorption of irrigating fluid during surgery leads to variations in blood chemistry. Both the groups showed similar results in the operative duration and          improvement of IPSS score .There was statistically significant decrease in the serum sodium and haemoglobin in the      postoperative period in Group B when compared to Group A.

Conclusion

            Our results demonstrated that bipolar resection in normal saline was found to be superior as it produced only  minimal effect on serum sodium and the haemoglobin when compared with monopolar resection of the prostate.
