Abstract

Objective-To compare the efficacy and safety of combination of Tadalafil and Tamsulosin versusTamsulosinalone as a medical expulsive therapy in lower ureteric stones.  Methods-A prospective randomized open-label trial including 111   patients in the Institute of Urology, Madras Medical College, Chennai from September 2014 to December 2016.111    patients with single lower ureteric calculus 5-10 mm were divided into study group (56 patients) and control group       (55 patients). Study Group patients were given tadalafil 10 mg with tamsulosin 0.4 mgwhereas control group patients were given tamsulosin 0.4 mg only at bedtime for 3 weeks.         End points like stone expulsion rate, time to stone expulsion, requirement of analgesics and adverse side effects were noted.Results- Mean stone size and mean distance of stone from vesico-ureteric junction was comparable. Stone        expulsion rate was significantly higher in study group.       Similarly mean expulsion time and mean analgesic use was significantly lower in study group. There was statistically  insignificant difference in adverse side effects between both groups. Conclusion- Combination of tadalafil with tamsulosinsignificantly increases lower ureteric stone expulsion with shorter time to expulsion and better control of pain as compared to tamsulosin alone. The combination is safe with only minimal increase in adverse effects.
