ETAPLASTIC BREAST CANCER WITH OSSEOUS DIFFERENTIATION A CASE REPORT

KARTHIKEYAN G

Abstract

Metaplastic breast carcinoma is a very rare neoplasm which contains mixture of carcinomatous (epithelial) and sarcomatous (mesenchymal) elements in variable proportion accounting for 0.2 to 1 of breast malignancy. Metaplastic carcinoma with osseous  differentiation is even rarer. The frequency of cartilaginous and osseous metaplasia in breast cancer is 0.03 to 0.2. A 55 years old female patient got admitted with complaints of  recurrent swelling in right breast. She was thoroughly evaluated. Mammogram showed heterogenous hypoechoic macro lobulated lesion. USG breast showed ill defined heteroechoic lesion in lower inner quadrant. FNAC came as malignant phyllodes. HPE of lumpectomy (previous surgery) specimen was Metaplastic carcinoma breast monophasic spindle cell variant with osseous metaplasia. Modified Radical Mastectomy was done. Histopathology came as metaplastic carcinoma breast with osseous differentiation, posterior         resected margin positive for tumour. Patient was treated with adjuvant chemotherapy and radiotherapy. In summary we report a case of metaplastic breast cancer with osseous metaplasia, a rare clinical entity.
