A RARE PRESENTATION OF MALIGNANT FIBROUS HISTIOCYTOMA

Abstract


Malignant fibrous histiocytoma also called as  pleomorphic undifferentiated sarcoma usually present as an extremity tumour or a retroperitoneal mass. Our patient Mr.sivasubramaniyum 68 yrs male presented with a history of swelling in left groin for 1 year. Swelling was partially   reducible. No other significant present or past history. On examination 65 cm firm, irregular mass felt in Left inguinal region extending from inguinal ligament to root of scrotum, cough impulse was present, swelling was partly reducible, and skin was not warm, not tender. Abdomen was soft. Basic investigations were normal chest X ray was normal, USG Abdomen showed left inguinal hernia with ill defined mass with mixed echogenicity present in Left inguinal region.  Clinical diagnosis of left irreducible hernia was made and planned for left inguinal exploration. Indirect sac was found and content was omentum. A tumour of size 65 cm was found in the wall of the sac near the deep ring. Tumour was excised in toto and sent for HPE. Hernioraphy was done. HPE   revealed malignant spindle cell tumor-high grade. IHC  revealed Malignant Fibrous Histiocytoma as the definitive diagnosis. we present this case a malignant fibrous  histiocytoma because of its rare presentation as inguinal  hernia.
