TRAUMA INDUCED HYPERGLYCEMIA OR DIABETES MELITUS

Abstract


Introduction and purpose Stress is known to cause diabetes mellitus. Road traffic accident especially with multi system injury can result in severe stress to the metabolic system. We retrospectively reviewed trauma patients to see if they developed post trauma diabetes mellitus. Materials and Methods We retrospectively collected data from trauma   register of patients who presented to us since October 2012. There were 731 patients in our database, at presentation ISS, comorbidities, host classification and GRBS values were noted. A random blood glucose level between 140mgdl to 200mgdl said to have impaired glucose tolerance and  associated with increased risk of developing type II diabetes mellitus. Those with values more than 200mg were                considered diabetic. Results and discussion Out of 731 patients,282 were excluded due to inadequate data and known diabetic patients. Out of 449 patients included in the study 343 were males (76.4) and 106 were females (23.6). 77 patients were considered as diabetic and their average ISS was 11.80. 137 patients were considered as impaired glucose tolerance and their average ISS was 11.63.ConclusionPrevalence of diabetes in India is 12. Our study shows that the incidence of newly detected diabetic in post trauma patients is as high as 17.15 and patients with impaired glucose tolerance was 30.51. Considering this high prevalence protocol for trauma patients should include                          baseline and followup blood glucose monitoring to avoid     potential complications related to diabetes.
