OPERATIVE TREATMENT OF HIP DISLOCATION IN CHILDREN WITH ARTHROGRYPOSIS MULTIPLEX CONGENITA THROUGH MEDIAL APPROACH A CASE SERIES

Abstract


INTRODUCTION-Hip involvement in  arthrogryposis occurs in 80 of cases and manifest either as contracture with or without dislocation. Hip dislocation occurs in 15 to 30 percentage and are stiff and irreducible. Unilateral dislocations surgically managed to prevent pelvic obliquity and scoliosis. The ambulatory potential decides treatment of bilateral dislocations. Two approaches are medial and  anterior. MATERIALS METHODS-During the period 2014-2015, we performed open reduction through medial approach in 5 patients with AMC. The median age at surgery was 8 months.There are three females and two males. There are three unilateral and two bilateral hips. The containment of the femoral head is assessed with radiographs and CT scan post operatively.RESULTS-We obtained good radiological and clinical outcome in all patients.Good range of movements in all patients.No redislocation at 6-8 months follow up.CONCLUSION-Early reduction through medial approach in AMC give good results.Bilateral hip dislocation can be  operated simultaneously. Reduced hip gives better hip           biomechanics and improves ambulation.
