Herpes Zoster Ophthalmicus occurs due to reactivation of varicella zoster virus affecting the trigeminal nerve ganglion. It can manifest by inflammation of any of the layers of the eye. Rarely it may cause ciliary ganglionitis and present with Adies pupil. Adies pupil is caused by damage to the postganglionic fibers of the parasympathetic innervation of the eye, it causes a dilated pupil not reacting to direct or consensual but tonic reaction to near reflex.we are presenting a case of herpes zoster ophthalmicus manifesting with Adies pupil.
