The case of a clinically significant Anti A1 - an argument for different phases of compatibility testing

Abstract


We present a case of A2B positive blood group with an anti A1 reactive at 37 degree Celsius detected during the Coombs phase of compatibility testing. A pretransfusion compatibility testing of the patient's blood at 37 degree Celsius and Coombs phase was incompatible with four AB positive blood groups while one AB positive unit and a B positive unit were found to be compatible. Further investigation using Anti A1 lectin revealed the patient's blood group to be A2B group with an Anti A1 reactive at 37 degree Celsius. This article highlights the need to be aware of such subgroups, where an anti A1 reactive at 37 degree Celsius can cause severe haemolysis. It is essential that compatibility testing at different phases be performed to rule out the presence of these irregular antibodies.
