Abstract

A 24 years old male presented with complaints of weakness and sensory disturbances of both lower limbs for the past 2 months with bladder incontinence for the past 3 days. Clinical examination revealed generalised wasting (more in the lower limbs), flaccid paraparesis with brisk reflexes and loss of all sensations over both lower limbs. MRI of s pine revealed ring enhancing lesion involving the thoracic cord with syrinx suggestive of Intramedullary Tubercuolma. Further evaluation revealed bilateral pleural effusion in Chest X ray. So we report this case as a rare initial presentation of CNS tuberculosis as Intramedullary Tuberculoma of the Thoracic cord presenting with flaccid paraparesis.
