A rare case report on metastases to Parotid from carcinoma esophagus

Abstract


Malignant neoplasms of the salivary glands are uncommon constituting approximately 35
of all head and neck malignancies and 0.51 of all cancers. Metastatic disease to the salivary glands
represents less than 10 of salivary gland tumors and it should be considered as one of the
differentials. The spread is usually hematogenous. Although rarely encountered, metastatic disease
to the salivary glands is widely reported in the medical literature as isolated case reports or small
series. The rich lymphoid content of the salivary glands makes an ideal ground for seeding of head
and neck primaries. Malignancies from distant infraclavicular sites also spread to the salivary glands,
usually to the submandibular glands and to a lesser extent the parotids. In the parotids, metastases
from head and neck malignancies, breast, kidney, upper gastrointestinal and lung primaries are the
most frequently reported ones. We are describing herein the case of an esophageal cancer with a
parotid nodal metastases. A 56 year old gentleman was diagnosed with squamous cell carcinoma in
the middle thoracic esophagus. He was found to have a progressive swelling in parotid region and
subsequently this was confirmed with FNAC from the parotid swelling as squamous cell carcinoma.
FDG PETCT revealed strong FDG-avid parotid node in the right parotid gland. The current case adds
another differential diagnosis of parotid swelling to the existing literature.
